Allows your CompHealth specialty team to begin the credentialing and search process.
Specialty Skills Checklists
Complete and send a skills form for each specialty you're qualified to practice in and want to pursue.

Supplemental Claim Form
Necessary for each malpractice claim you've experienced. Include/attach all third-party documentation for malpractice claims (complaint/answer/judgment).
Health Statement
Complete this form regarding your health and recent vaccinations.
Background Check Authorization
Authorizes a criminal background check to be processed.
