Current Resume — Please list each facility where you have worked, including city and state, dates of employment, and education information. Include the facility trauma level, if applicable. Note: In order to expedite the process, please include dates in month/year format. If you cannot recall exact dates, please estimate.
Skills Checklist — Complete the Skills Checklist, making certain to include current state licenses (if applicable) and equipment experience. Please sign, initial and date at the indicated locations at the bottom of the page(s).
Current certifications — Copies of your current Registry Certifications, such as ARRT, ARDMS, NMTCB, etc. Please include MQSA documents, if applicable.
