Allows your CompHealth specialty team to begin the credentialing and search process.
Specialty Skills Checklist
Indicates the specialty in which you are qualified to practice and that you want to pursue.
Supplemental Claim Form
Necessary for each malpractice claim you've experienced. Include/attach all third-party documentation for malpractice claims (complaint/answer/judgment).
Health Statement
Please complete this form regarding your health and recent vaccinations.
