The CompHealth application process includes the initial application, a skills checklist and other credentialing materials. The forms you will need to complete the application process are provided below.

You will need to complete the application and forms below throughout the process. Sign each where requested. Then, fax or mail them to CompHealth.
Getting Started
Getting started is easy. Just send us copies of the four documents listed below. Should you have any questions, please contact CompHealth at .
- Current Resume — Please list each facility where you have worked, including city and state, dates of employment, and education information. Include the facility trauma level, if applicable. Note: In order to expedite the process, please include dates in month/year format. If you cannot recall exact dates, please estimate.
- Skills Checklist — Complete the Dosimetrist Skills Checklist, making certain to include current state licenses (if applicable) and equipment experience. Please sign, initial and date at the indicated locations at the bottom of the page(s).
- Current certifications — Copies of your current Registry Certifications, such as ARRT, ARDMS, NMTCB, etc. Please include MQSA documents, if applicable.
- Current CPR card
